HRSA Community Health Quality Recognition

On behalf of Health Resources and Services Administration’s (HRSA) Bureau of Primary Health Care, Tandem Health was presented with the 2021 Community Health Quality Recognition certificate and badge for the center’s hard work in 2021.

The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent Uniform Data Systems (UDS) reporting period.
