Your First Visit
Establishing your care with Tandem Health is easy. Just contact us and then complete our new patient packet, which can be mailed to you or you may download the medical patient packet in English or Espanol. If you are a new dental patient, please download the packet in English or in Espanol.
Bring your completed packet, along with the items on the registration checklist, with you to your appointment. We take it from there!
Registration Checklist
- Government-issued photo ID
- Insurance card
- Insurance co-pay or a nominal fee
- Proof of address
- Sliding Fee Application (click here to open)

- Immunization records
- Medical records
Plan to arrive at least 15 minutes before your scheduled appointment giving us time to complete your registration with our practice. After your appointment, we’ll help you navigate next steps whether it’s getting a prescription filled, scheduling follow-up care, a specialist referral or testing.
